You have taken the medication. You have tried the injections. You have gone through physical therapy, stretching, rest, and exercises that promised to solve the problem.

The pain improved for a while. Then you sat too long, drove too far, lifted something, slept in the wrong position, or simply woke up one morning and felt the burning, tingling, or shooting pain return.

Now you are asking the question so many of my clients ask: “Why does my sciatica keep coming back?”

First, I want you to hear this clearly: YOU ARE NOT BROKEN. YOU ARE NOT CRAZY. You are not failing because your pain keeps returning.

You may simply have been shown the wrong direction.

The painful side may not be the side causing the cycle

When your right leg hurts, where does everyone look? The right leg. The right hip. The right buttock. The right side of your lower back.

That seems logical. But sciatica is rarely just a painful-leg problem. Your body is one connected system. When one side becomes guarded, weak, or overloaded, the opposite side often compensates.

That compensation can continue long after the original flare begins to settle.

This is the part that is foreign to most people: the painful side may be the side reporting the problem, not the side that needs more stretching, pressure, or strengthening.

If you keep attacking the painful side, you may calm the symptoms temporarily while leaving the imbalance underneath. Then the pain returns because the body has returned to the same pattern.

That is why I work 180° different. I look at THE OPPOSITE SIDE.

Why medication and injections often provide temporary relief

Medication can reduce pain signals. Injections can reduce inflammation around an irritated nerve. That relief can be important, especially during a severe flare.

BUT relief is not the same as correction.

An injection does not change the way one side of your body has been compensating for the other. Medication does not teach your hips, pelvis, and spine to share movement evenly.

So when the medication wears off or the injection stops working, the same imbalance remains. You sit, walk, bend, or drive in the same pattern, and the sciatic nerve gets irritated again.

That is not a personal failure. It is an incomplete approach.

Research also shows that recurrence is common. In one prospective study of people whose leg pain had resolved after nonsurgical care for a lumbar disc herniation, 25% experienced recurrent leg pain within one year. You can read the published recurrence study here.

The message is simple: feeling better for a few weeks does not prove the underlying pattern has changed.

Why standard physical therapy may not stop the return of sciatica

Physical therapy is not one single method. The results depend on what is being assessed, what is being strengthened, and which side is being worked.

Many people are given general stretches for the painful hamstring, piriformis, hip, or lower back. Others are given exercises designed to strengthen the painful side.

IF the painful side is already working too hard, adding more pressure there is not the answer.

IF the non-painful side has stopped carrying its share, ignoring it leaves the body in the same compensatory position.

And IF you stop exercising the moment your pain quiets down, the imbalance remains ready to trigger another flare (sometimes within days, sometimes after several months).

This is why the second phase of my system matters so much. You do not stop when the pain decreases. You continue until your body can hold the balanced position during walking, sitting, driving, sleeping, and normal daily activities.

The 180° reversal: work the opposite side first

After 17 years as a traditional Physical Therapist, I had to face an uncomfortable truth. I was doing what I had been taught to do. I was treating the painful side. And 80-85% of my clients were not getting lasting results.

The problem was not their effort. The problem was the direction.

When I began working the non-painful side and balancing the body’s compensatory patterns, my success rate rose to 95-97%. Since then, I have helped more than 10,000 sciatica sufferers, including over 5,200 people online since 2018.

Balance → Stabilize → Return

Stabilize

Return

Finally, you learn maintenance techniques that help you return to normal activities and protect your progress. You return to walking, traveling, driving, playing with your family, golfing, or simply getting through your day without planning every movement around your leg.

The purpose is not ongoing dependency. The purpose is self-assessment, self-correction, and confidence in your own body.

How to tell whether your body is responding

You do not need to wait eight weeks to learn whether an approach is doing anything. Your body gives you signals.

During the first 15-20 seconds of a properly selected movement, pay attention to the direction of your symptoms.

Positive signs include:

Negative signs include increasing weakness, spreading numbness, or symptoms moving farther down the leg. IF those signs continue or worsen, stop pushing through them and seek an appropriate medical evaluation. Severe or progressive weakness, loss of bladder or bowel control, or numbness around the groin requires urgent medical attention.

The direction of your symptoms matters. Do not judge progress only by the pain number.

Do not commit to another promise. Test the opposite approach
