I know exactly where you are right now. You are sitting on the edge of your chair, or maybe lying flat on the cold kitchen floor, wondering if this constant, burning agony in your leg WILL ever actually stop. You have probably seen the doctors. You have probably done the six weeks of physical therapy for sciatica. You have probably had the cortisone injections that worked for three days and then quit. And after all that effort, you are still in pain. You feel broken. You feel like your body has betrayed you, and worse, you feel like the experts do not have a clear answer for what to do next.

BUT I have a simple true thing to tell you: you are not broken. You are not crazy. And you are certainly not asking for too much by wanting your life back. The reason you are still suffering is not because your sciatica cannot improve. It is because everything you have been told to do has been backwards.

What follows is general education about sciatica, not medical advice or a personal diagnosis. If your symptoms are severe, worsening, or affecting your bladder, bowel, or strength, speak with a qualified healthcare professional.

If you tried physical therapy for sciatica and it did not last, the problem may not be you, it may be the approach. Traditional physical therapy usually focuses on the painful side. Dean Volk's 180° approach works THE OPPOSITE SIDE first.

Why physical therapy for sciatica often falls short

For more than 34 years, I have worked in sciatica education and coaching, and for the first 17 of those years I followed the standard approach. I followed the clinical guidelines. I focused on the painful side. I told my clients to stretch their tight hamstrings, strengthen their core, and do their nerve glides on the leg that was already screaming at them. And do you know what happened? Under the traditional model, only 15-20% of clients got lasting results.

That does not mean individual physical therapists are bad at what they do. It means the traditional site-of-pain premise is incomplete for chronic sciatica. IF your left leg hurts, the standard approach usually goes after the left leg. It sounds logical, BUT in the world of chronic sciatica, that logic often keeps you trapped.

When you stretch or load tissue around an already irritated nerve, you can aggravate it instead of calming it down. That is why you can leave the PT clinic feeling okay for an hour, then have the pain roar back the moment you get into your car. The traditional approach often addresses the smoke, BUT it does not always put out the fire. At Sciatica Relief Now LLC, we do things 180° different.

The reversal most people never hear

IF you have tried everything and nothing has worked, it is time to stop trusting promises and start testing proof. The 180° Sciatica Solution is built on a discovery that changed everything in my work. In my own work, 95-97% of clients report meaningful change (self-reported); results vary. After seeing thousands of people fail with the standard approach, I realized the answer was not on the painful side at all. The answer is on THE OPPOSITE SIDE.

Your body is a master of compensation. When one part of you is out of balance, the rest of your system shifts to protect it. Over time, those compensation patterns become the real culprit. They pull your spine and pelvis into positions that keep pressure on the sciatic nerve. By the time you feel the pain in your leg, your whole system may already be skewed.

My 180° Solution focuses on what I call body balancing. Instead of attacking the side that already hurts, we work the non-painful side first to restore balance to the pelvis and spine. This gives the irritated nerve a chance to calm down so your body can heal on its own. It is a non-medicinal, non-invasive sciatica treatment approach that has helped over 10,000 sufferers, including over 5,200 online since 2018, reclaim their lives.

What works after PT did not last

The next step is not more random stretching. The next step is the right sequence: Balance → Stabilize → Return. Most clients see meaningful change within 8 weeks. Timelines vary by person. On average, people report a 50% improvement in 3 to 5 weeks, and 85% improvement by 8 weeks. Results vary.

1. BALANCE. This is the foundation. We use specific movements on the non-painful side to re-align your body. IF your body is not balanced, no amount of strengthening WILL hold. We must stop the irritation first.

2. STABILIZE. Once your pain starts to recede, we lock in that new balanced position. We teach your muscles how to hold the right posture so the pressure does not come back.

3. RETURN. This is where you reclaim your life. We show you how to maintain your results so you can get back to the things you love: playing with your grandkids, walking on the beach, traveling, golfing, or driving without that burning in your glute and leg.

Why this comparison is fair, and why it matters to you

I am not telling you every clinic gets it wrong because the clinicians do not care. I am telling you the standard direction of care is often backwards for chronic sciatica. That distinction matters. Good people can still be taught a flawed premise. I know because I spent 17 years doing it the traditional way.

Once I stopped chasing symptoms on the painful side and started correcting compensation from THE OPPOSITE SIDE, everything changed. In my own work, 95-97% of clients report meaningful change (self-reported); results vary. Since then, I have helped 10,000+ sciatica sufferers break this problem, including people who suffered for 3 months and people who suffered for 63 years.

What evidence says, and what it does not say

You have probably been told your MRI explains everything. BUT imaging does not always match pain. A landmark study by Jensen et al. found that many people without low back pain still had disc bulges or protrusions on MRI, which is one reason scans alone do not tell the whole story. That matters when you have been made to feel like your spine is doomed.

You also need to know this: surgery is not usually the first step unless there are urgent red flags. Major guidance supports conservative care before surgery for most cases of lumbar disc herniation with radicular pain.

That does not prove every exercise should be done my way. What it DOES show is that the common story, pain on the scan, treat the painful spot, rush to the next procedure, is often too simplistic. IF what you tried did not last, you need a different lens.

Stop trusting promises. Start testing proof.

I do not want you to commit to a program IF your body does not respond. That is why I created the FREE 3-Day Sciatica Proof Lab. It is a highly personalized 3-day series of 1:1 Zoom sessions with me, 30 minutes each, where I listen to your frustrations and manually guide you through body balancing movements based on what your body is doing.

This is not a generic class. It is not a one-size-fits-all video library. It is direct coaching. Over 3 days, we prove to you and your body whether this 180° different approach is the missing piece. No commitment IF your body does not respond.

IF you are at your wits' end, IF physical therapy for sciatica did not last, and IF you are ready to see why everything you have been told has been backwards, this is your next step.

FAQ: physical therapy for sciatica

Why does physical therapy not help sciatica?

Because it often focuses on the site of pain instead of the pattern causing the pain. IF the painful side is being irritated over and over, stretching and strengthening that side first can keep you stuck.

Should I stretch the side that hurts?

Not first. In my experience, stretching the painful side too early is one of the biggest reasons sciatica keeps coming back. I start with THE OPPOSITE SIDE to balance the body before I add strengthening.

How is the 180° approach different from physical therapy?

The key difference is direction. Traditional physical therapy for sciatica usually goes after the painful side. My 180° approach starts with body balancing on the non-painful side, then moves through Balance → Stabilize → Return so you can self-assess, self-correct, and reclaim your life.

Can sciatica improve without surgery?

In many cases, yes. Conservative care is widely recommended before surgery in the absence of urgent red flags, and I have helped clients from 3 months to 63 years of suffering break sciatica without meds, injections, or surgery.

Your next step to reclaim your life

You have two choices right now. You can keep doing what you have been doing, stretching the painful side, hoping the next injection is the one, and watching your life get smaller. Or, you can take the 180° turn.
