You bend over to tie your shoe, and there it is again.
That familiar bolt of pain shoots through your back, hip, or leg. You freeze. You hold your breath. Then the sinking feeling arrives: “Not again.”
You thought you had finally turned the corner. You rested. You stretched. You strengthened. Maybe you tried medication, injections, chiropractic care, or physical therapy. The pain settled down, and you started walking, driving, sleeping, or playing with your family again.
Then sciatica came back.
If this has happened more than once, I know how defeated you feel. You may feel betrayed by your own body. You may be exhausted, skeptical, and at your wit’s end. You may even believe that this is simply what your future looks like.
You are not broken. You are not crazy. You are not asking for too much by wanting to sleep through the night, walk without fear, play with your grandkids, golf, play pickleball, travel, or drive without constantly calculating how long you can sit.
But here is the hard truth: relief is not always recovery.
When the pain goes away but the imbalance stays
Pain relief and body balance are not the same thing.
Pain is often the alarm. When the alarm becomes quiet, that does not automatically mean the reason for the alarm has been corrected. Your body may simply have found a temporary way to reduce pressure and protect itself.
That is why you can feel better for a few days, weeks, or months and then experience another flare-up. The same movement pattern, weight shift, or side-to-side imbalance is still waiting underneath the surface.
The flare-up often does not come from a brand-new injury. It comes from the same old imbalance showing up again when your body meets a new load – bending to tie a shoe, lifting a grandchild, sitting through a long drive, walking farther than usual, or returning to exercise too quickly.
Relief is a pause. Balance is the fix that creates long-lasting relief leading to healing.
This is where everything you have been told may be backwards. Most approaches chase the painful side because that is where the symptoms are. But the painful side is often the guarded, overprotective side. It is reacting to the imbalance, not necessarily creating it.

Why working only on the painful side can keep the cycle going
When one side hurts, your brain immediately changes how you move. You shift weight. You shorten your step. You tighten certain muscles. You stand differently. You sit differently.
The painful side becomes protective. The opposite side often takes on more of the work. It may become tighter, overloaded, or responsible for keeping you upright. That creates a compensation pattern across your hips, pelvis, lower back, and legs.
IF you stretch and strengthen only the painful side, you may be working on the area that is already bracing for protection. IF you keep loading that side without checking the opposite side, you can reinforce the very pattern that allows sciatica to return.
This does not mean your doctor, therapist, or other healthcare professional was careless. They were following the direction that seemed logical: treat the area that hurts. The problem is the direction, not the person.
My approach is 180° different. I look at the whole pattern and pay close attention to the side that does not hurt. The answer is often on THE OPPOSITE SIDE.
That is the foundation of the 180 degree sciatica solution: stop assuming the painful side is the side that needs the most attention. Find the imbalance first. Then correct it in the right order.
Your next step starts with a simple self-assessment
Do this today in front of a mirror, or ask someone you trust to watch you stand and walk. Do not force any movement. You are looking for differences, not trying to prove anything.
1. Notice which side feels tighter.
Stand comfortably and compare both sides of your hips, glutes, hamstrings, and lower back. Which side feels more restricted? Which side feels like it is doing more work?
2. Check whether one hip rides higher.
Place your hands on the top of your hip bones. Does one side sit higher than the other? Do you naturally lean away from the painful side?
3. Watch your feet when you walk.
Take several easy steps. Does one foot turn out more? Is one step shorter? Do you avoid pushing off one leg?
4. Notice which leg does all the work when you stand.
Stand for 15-20 seconds. Do you shift your weight onto one leg? Does one knee lock while the other stays soft? Does your body lean to one side?
These signs do not give you a diagnosis. They give you a direction to investigate. IF your body consistently shifts away from the painful side, the opposite side may be carrying more responsibility than you realize.
Test the opposite side carefully
Here is a simple experiment. Stand near a countertop or sturdy chair for support. Keep the movement easy and controlled. Shift your attention to the non-painful side and gently lengthen or activate that side for 15-20 seconds. This may involve a light standing stretch, a gentle weight shift, or a small controlled movement that does not reproduce sharp or increasing leg pain.
Then stand normally and notice whether your posture, step, or sense of pressure changes.
Do not push through worsening pain, numbness, or tingling. The point is not to force a stretch. The point is to observe whether changing the opposite side changes how your body feels. That response is valuable information.
The 3-step framework that stops the comeback
The 180° Sciatica Solution follows a clear sequence: Balance -> Stabilize -> Return.
Most people who say “my sciatica keeps coming back” received some version of the first step, felt better, and stopped there. But steps two and three are what help the change hold when you return to real life.
Balance
First, you balance the body by identifying the compensation pattern and correcting the side-to-side imbalance. This is where I often work with the non-painful side to reduce the burden on the painful side.
You are not simply chasing pain. You are restoring a better position so your body does not keep sending the same alarm.
Stabilize
Once your body is balanced, you use custom strengthening exercises for sciatica to stabilize that position. This is not random strengthening. It is strengthening that matches your new balance.
You build support through the core, hips, and legs so your body can hold the improved position during standing, walking, sitting, lifting, and sleeping. IF you skip this step, your body can drift back toward the pattern that created the flare-up.
Return
Finally, you learn how to return to the activities that matter to you. That includes walking longer, driving, traveling, playing with family, golfing, playing pickleball, or getting back to the activities you stopped trusting.
You do not jump from temporary relief to 100% effort overnight. You increase your activity in measured steps and pay attention to early warning signs such as one-sided tightness, shifting weight, or a shorter stride.
Balance -> Stabilize -> Return. That is how you stop treating relief as the finish line.

What I learned after more than 34 years in this work
Dean Volk, MPT, is an International Sciatica Consultant with more than 34 years of experience in sciatica education and coaching.
For my first 17 years I worked inside the traditional model, following the standard approach and treating the painful side. Under the traditional model, only 15-20% of clients got lasting results. That was difficult to accept because my clients were working hard. They were doing what they were told. They were not failing. The direction was wrong.
When I began working with the non-painful side and correcting the body imbalance first, everything changed. In my own work, 95-97% of clients report meaningful change (self-reported); results vary.
I have worked with clients who suffered for three months and clients who suffered for 63 years. On average, people report a 50% improvement in 3 to 5 weeks, and 85% improvement by 8 weeks. Results vary. Most clients see meaningful change within 8 weeks. Timelines vary by person, and your body must respond to the approach.
My clients are not looking for another temporary break from pain. They want to reclaim their life. They want to get out of the chair when their grandkids arrive. They want to sleep through the night. They want to take a trip without planning every stop around their back and leg.
That is why I do not want you to commit to a program IF your body does not respond. Test the approach first.
Do not wait for the next flare-up
IF your sciatica keeps coming back, do not assume you need to start over from the beginning. First, ask a better question: “What imbalance did I never correct?”
Then take these next steps:
– Stop automatically stretching or strengthening only the painful side.
– Observe your standing, walking, and weight-shifting patterns.
– Test the opposite side gently for 15-20 seconds at a time.
– Do not move into strengthening exercises for sciatica until your body is balanced enough to use them correctly.
– Progress through Balance -> Stabilize -> Return instead of stopping when the alarm gets quiet.
– Seek urgent medical attention for new bowel or bladder changes, numbness in the saddle area, or rapidly worsening leg weakness.
You do not need to believe me because I have more than 34 years of experience. You need to test what your own body does when you finally look in the opposite direction.
