IF you are reading this, I know exactly where you are. You are likely sitting on the edge of your chair — or maybe lying flat on the floor because it is the only place you can find a moment of peace — wondering IF this lightning bolt of pain will ever actually go away. I want you to know right now: you are not broken, and you are not crazy. I hear you, and I understand the absolute frustration of doing "everything right" and still feeling like your leg is on fire!!
Here is the short answer: sciatica often keeps coming back because you are resting too long, stretching the painful side, pushing through nerve pain, masking symptoms without fixing the cause, ignoring body imbalance, strengthening too early, or returning to normal activity too fast. I know that sounds blunt, BUT after 34+ years of experience, helping 10,000+ sciatica sufferers, including 5,200+ online since 2018, I can tell you this with confidence — most people are recovering backwards.
My name is Dean Volk, MPT. I am a licensed physical therapist who spent 17 years inside the traditional model before developing the 180° Sciatica Solution. Today my work focuses on sciatica education and coaching. Over the last 17+ years, I have helped 10,000+ sciatica sufferers break sciatica by doing things 180° different, with a 95-97% success rate. 50% of clients see major change by weeks 3-5 (self-reported); results vary. Most of those people came to me after they had already tried everything — pills, shots, generic exercises, even traditional Physical Therapy. The reason they were still hurting was simple: they were making the same seven mistakes that I see over and over again.
Mistake 1: Staying in bed too long
I know it sounds logical. Your back hurts, your leg is numb, and every movement feels like a gamble. Your first instinct is to crawl into bed and stay there until the pain disappears. BUT this is one of the biggest mistakes you can make. When you remain completely immobile, your muscles begin to stiffen, your circulation slows down, and your discs do not get the nutrient flow they need to heal. Prolonged rest often slows sciatica recovery because your body starts to lock up in a guarded position.
The fix: you need to move, BUT you need to move wisely. I tell my clients that movement is medicine IF the dosage is right. Instead of staying in bed for three days, get up every 30-45 minutes and walk for two or three minutes (even if it is just around your living room). This keeps blood flowing and prevents the secondary stiffness that often hurts worse than the original sciatica.
Mistake 2: Stretching the painful side
This is a big one. Almost everyone I work with starts out by trying to stretch out the pain. You feel tightness in your glute or hamstring, so you pull your knee to your chest or try to touch your toes. Stop. IF your sciatic nerve is inflamed, it is like a raw, irritated piece of thread. Every time you stretch it, you can tension that nerve and make the irritation worse. You might feel a little relief for 30 seconds because of the stretch reflex, BUT ten minutes later the pain comes back even stronger.
The fix: this is where we use the first step of my framework — Balance. Often, the answer is not stretching the side that hurts. It is working on the opposite side to bring your pelvis back into alignment. That is what makes my approach 180° different. I have seen clients get major relief just by shifting the focus away from the painful side. If this sounds foreign to most of what you have been told, good — because most of what you have been told is backwards. You can also read more here: What If Stretching the Wrong Side Was Actually the Secret to Stopping Your Sciatica.

Mistake 3: Pushing through nerve pain
You have probably heard "no pain, no gain" your whole life. In the world of sciatica, that mindset can keep you stuck. Nerve pain is not the same as muscle soreness. You do not push through it. IF an exercise spikes your leg pain, your body is telling you that the movement is wrong for right now. Pushing through can keep your nervous system on high alert and reinforce the exact pattern you are trying to break.
The fix: listen to your body and watch the pattern of your symptoms. IF pain travels further down your leg, that is a bad sign. We want the pain to centralize, meaning it moves out of the foot or calf and back up toward the lower back. That is often a sign you are moving in the right direction.
Mistake 4: Relying on pills or shots as the answer
I am not saying medication does not have a place, BUT it is not the answer. Whether it is an anti-inflammatory, a nerve blocker, or a steroid injection, those options turn down the volume of the alarm. They do not fix why the alarm is going off in the first place. I have seen so many people feel better after a shot, go out and garden or lift something heavy, and end up right back where they started because they could not feel their body's warning signals.
The fix: use medication only to calm symptoms enough so you can do the right corrective work. Do not confuse less pain with real healing. You still need to address the imbalance that caused the sciatica in the first place. That is why my system follows Balance → Stabilize → Return. We do not just cover the smoke. We go after the fire.
Mistake 5: Ignoring why your body keeps getting pulled out of position
Most people focus only on the disc or the nerve. They look at an MRI and say, "I have a bulge at L5-S1, that is my problem." BUT the better question is this: why did that disc bulge? IF you have a pelvic tilt, rotational imbalance, or uneven loading pattern in your hips, you are putting pressure on the same structures every time you stand, sit, or walk. IF you do not correct that, the pain WILL keep coming back.
The fix: this is the heart of Break sciatica. We start with Balance. We look at how you stand, how you sit, how your weight shifts, and what the opposite side is doing. Once you find that more neutral position, you finally give the nerve room to calm down. If you want to go deeper on this, read What If The Reason Your Sciatica Keeps Coming Back Is Because You're Sitting On The Wrong Side?.
Mistake 6: Strengthening before you balance
I see this in the traditional model all the time. You go in with sciatica, and they immediately have you doing bridges, bird-dogs, and planks. BUT IF your body is out of balance, you are just strengthening the wrong pattern. You are locking in a crooked position. That is why so many sciatica exercises fail.
The fix: now we move into Stabilize. Notice the order. We do not stabilize UNTIL we have balanced the body first. Once you find the position that reduces your symptoms, THEN you use targeted strengthening exercises to hold that position. This is a total 180° different approach from what most clinics do, and it is a big reason my clients get better when other methods have failed. For more on this, read What If The Reason Your Sciatica Keeps Coming Back Is Because You're Only Treating The Painful Side? and What If Your Sciatica Isn't Getting Better Because You're Strengthening the Wrong Side?.
Mistake 7: Going back to normal too fast
The moment your pain drops from a 9 to a 2, it is tempting to go right back to the gym, pick up your grandkids, or take a long hike. This is the danger zone. Just because the pain is lower does not mean your body is ready for full speed. Returning to high-demand activity too fast is one of the biggest reasons sciatica keeps coming back.
The fix: this is the Return phase. You go back gradually and intelligently. I guide my clients through a progressive return so we can test how the body responds to 15-20 minutes of activity before trying an hour. That is how you Reclaim your life without ending up right back at square one.
I know how exhausting this journey is. I know you are tired of generic advice that keeps letting you down. IF you have tried everything and nothing has lasted, that does not mean you are beyond help. It usually means you have been treated backwards.
That is exactly why I built the 180° Sciatica Solution around Balance → Stabilize → Return. First, we balance the body by working the opposite side. Then we stabilize that new position with custom strengthening exercises. Then we help you return to normal life without falling back into the same pattern. That is how you Break sciatica and Reclaim your life. FAQ
### What makes sciatica worse?
Sciatica often gets worse when you stay in bed too long, stretch the painful side, sit unevenly, push through nerve pain, or go back to activity too fast. IF symptoms are getting stronger, spreading further down your leg, or affecting bowel or bladder function, seek medical care right away.
### Should I rest or stay active with sciatica?
Short-term rest can calm things down for a brief window, BUT too much rest usually makes sciatica worse. In most cases, smart movement is better than complete bed rest. Small walking breaks every 30-45 minutes are often a good place to start.
### Why does my sciatica keep coming back?
Because the cause usually was never corrected. I see this all the time. People focus on the painful side, the symptoms, or the MRI finding, BUT they never address the body imbalance driving the irritation. That is why the 180° different approach matters so much.
